The Treatment of Non-union and Avascular Necrosis Following Subcapital Fracture of Femur by Nailing and Cortical Bone Graft


Abstract
A case report is made on a 46 year old male who had one year old subcapital fracture of his right femur with the complication of non-union and partial avascular necrosis of femoral head.
Initially he was attempted to have hip-nailing one week after the injury, however, it was unable because of his general condition became worse just after the skin incision. He had hip spica cast for a month after that and since then he was never able to walk. His right leg has developed muscle atrophy, shortening and pain in his hip with limitation of motion.
He was received McLaughlin type hip-nailing and cortical bone graft that was 8.3cm (341 inch) long and 0. 7cm in diameter. He started his leg exercise from the next day of surgery and full weight bearing was began at ten months after operation.
At the 14 months following surgery, the X-ray showed complete bony union with integration of grafted bone at proximal a half and the density of femoral head due to avascular necrosis became decreased in well preserving hip joint space. He was pain free on walking and no any difficulties with his right hip for daily living.
